TO REDEEM PREVENTIVE FREE GOODS: All free goods must be of equal or lesser value than least expensive product purchased. Purchase must be made from an authorized Dentsply Sirona Dealer between 4/1/2023 and 6/30/2023. Purchase must be made on ONE invoice. Rebate and/or free good requests, including all required supporting documentation (e.g., Invoices) must be received by Dentsply Sirona no later than 7/31/2023. For Preventive promotions, note addresses below. Late or Incomplete submissions will not be processed and no rebate or free goods will be issued. Customers should keep copies of all rebate submission documents. All requests for free goods and rebates are subject to Dentsply Sirona final review and approval. Limit 5 redemptions of each offer per Practice. Offers valid in the 50 United States. These offers may not be combined with any other Dentsply Sirona offers, contracts, or formulary agreements. Applicable tax, if any, is the sole responsibility of the customer. Dentsply Sirona reserves the right, in its sole discretion, to terminate the promotion at any time, for any reason. Cancellation or return of the required purchased product(s) will lead to revocation of the offer and forfeiture of the promotional offering.
